1. Records Request Form - This document starts the order process when ordering medical records and provides the pertinent information.
2. Medical Memorandum - This Memorandum is prepared reviewing one's medical records and is utilized to ascertain the physical condition of the patient both before and after an accident/injury. The Memorandum may contain personal opinions or conclusions at the end of the Memorandum with regard to the extent of injuries.
3. Medical Chronology - The Medical Chronology contains concise data that is obtain strictly from the patient's medical records, and does not contain any personal opinions by the author - only those reflected in the medical records.
